Public Records Request - Download
Union Public Schools recognizes that the public has a right to be fully informed concerning its operations. The district strongly believes that informed citizens are vital to the successful functioning of the democratic process which the district desires to exemplify to its students.
In order to achieve these goals, the Board hereby states that all records of the district, except those designated as confidential in this policy statement, shall be open to any person for inspection, copying and/or mechanical reproduction during regular business hours. All persons requesting the right to inspect non-confidential records of the district shall be accorded prompt access to those records.

Publicity Opt Out Form - Download
It has been the practice of the Union Public School district to use student names in yearbooks, student honor rolls, athletic programs, as well as in electronic and print publications. It has also been the practice of the district to cooperate with law enforcement agencies and the Department of Human Services; to provide student names and addresses to institutions of post-secondary education who desire to notify students of educational and scholarship opportunities; and to the armed services for recruitment purposes. The district will continue to disclose student information for these purposes and other similar educational purposes unless parents object in writing (via the form) and to the use of the student information in this manner.
Once the form is completed, a copy should be kept at the school site and another should be sent to the executive director of communications, Union Public Schools, 8506 E. 61st Street, Tulsa, OK 74133.
Talent Release Form - Download
Union Public Schools routinely takes photos of students for publications, videos, and for use on the Internet; and students may be routinely identified in these publications, as well as in print and/or in broadcast news stories. The Talent Release form, in essence a permission slip, may be used when there is advance notice of the taping, photo shoot, interview, etc., and should be used before publicly identifying students in Special Education or Alternative Education programs.
